Home medication reviews of diabetes and transplant patients can improve pharmaceutical care and prevent drug-related problems, research has confirmed.
A study by Swiss researchers, published in the International Journal of Clinical Pharmacy, identified the valuable role that pharmacist home medication reviews (HMRs) could play in these patient groups.
Fifth-year pharmacy students from 79 Swiss pharmacies visited patients as part of the study. In total, 22 reviews of transplant patients and 54 of patients with type 2 diabetes were completed.
The students identified 7.4 (± 2) drug-related problems per visited patient, with the type 2 diabetes patients having, on average, more of these problems (7.8 per patient) than those who had transplants (6.3 per patient).
